Eligibility Requirements: 

• A resident of Lancaster or York County

• Ages 18 to 64

• Have NO health insurance coverage to include covered by Medicaid/Medicare

• Have a household income 250% or below of the Federal Poverty Index Range

• No other Medical Provider for Primary Care

• A valid government issued picture ID

• Any bill/piece of mail in the patient's name that comes to the residence and is dated within the last month

• Monthly income for each adult living in the household dated within the past month.
